To obtain a copy of your Food Handler certificate please complete the Food Handler Certificate Replacement Form and enclose the $5.00 replacement fee.
To receive an ORIGINAL duplicate ServSafe® Food Manager certificate you must submit a Certificate Request Form located on the ServSafe® website and pay $20.00. There are detailed instructions to follow at:

Texas Food Manager Certificates
Texas State Health Services no longer provides Food Manager Certification. If you were certified since 2008 and need a duplicate certificate please contact the Texas State Health Services Department at 512.834.6727.
If you have lost your TABC Seller Training Certificate, you can access your information on TABC’s website and print out proof of certification. Visit the TABC website and click on the”Certificate Inquiry” link.
