Effective April 24, 2008, the fee to Replace a Texas Department of Health Certificate is $15.
Certificates that were issued between May 6, 2004 and April 24, 2008 are valid for 6 years but must be renewed every 2 years. You do not need to re-take the exam. You can mail in the renewal form up to 60 days before the expiration date. The fee to renew is still $10.
To Replace or Renew, print the following application and send it to the Texas Department of State Health Services with the required fee.
